Office of the Vice President Requirement for Cancer Treatment or Therapy Assistance
The list of Requirements for Chemotherapy, Radiation Therapy, Brachy Therapy, and Radio-Iodine Therapy Assistance from the Office of the Vice President below which was found posted at one of the Barangay Hall front desk in Davao City to help people know what they have to bring and present when applying for medical financial assistance for cancer treatment.
REQUIREMENTS:
- Application Form
- Referral Letter from the hospital / dialysis center (if not yet a partner service provider)
- Medical Records
(clinical abstract or medical certificate with complete name, signature and license of doctor)
(date should not be beyond six (6) months from the date of assessment) - Social Case Study Report or Barangay Certificate of Indigency
(addressed to the Office of the Vice President or to another government agency)
(it should be signed but the Barangay Captain or social worker) - Treatment Protocol
(there should be a computation of expenses, with complete name and signature of doctor)
(date should not be beyond six (6) months from the date of assessment) - Valid IDs of the patient and representative (front and back)
The Office of the Vice President (OVP) has a Davao satellite office located at the right side of Orchard Hotel building along J.P. Laurel Avenue, Bajada just right in front of the Davao City Water District (DCWD) Bajada office which is beside Robinson’s Cybergate.
There is usually a long line already early in the morning outside the office of people most of them asking or applying for financial assistance from the OVP. Best to come early, bring snacks and water in case while waiting in line.
