Please do not forget to bring your Vaccination card, own ballpen and your Safe Davao QR Code. In case you lost your vaccination card, please secure an affidavit of loss.

Also bring drinking water and snacks just in case.

Get a good rest and night sleep as well as eat your breakfast before going to the vaccination site/ center.

Wear your face mask and face shield properly. Sanitize and observe social distancing at all times.
